Registered Nurse - Home Care

We are looking for compassionate and skilled Registered Nurses to be part of the dynamic team.

Key Responsibilities :

• Patient Assessment : Conduct comprehensive health assessments, including monitoring vital signs and identifying any changes in patient conditions.
• Care Planning : Develop and implement personalized care plans tailored to each patient's needs and medical conditions.
• Medication Administration : Safely administer prescribed medications and monitor for potential side effects.
• Wound Care and Medical Procedures : Perform advanced nursing tasks such as wound dressing, IV therapy, catheter care, and injections.
• Health Education : Educate patients and families about disease management, medication use, and healthy lifestyle practices.
• Emergency Response : Act promptly in medical emergencies, providing first aid or resuscitation if necessary.
• Collaboration : Coordinate with physicians, therapists, and other healthcare professionals to ensure seamless care delivery.
• Documentation : Maintain accurate records of patient care, including progress notes, treatment plans, and updates.
• Infection Control : Adhere to strict infection control practices to protect patients and caregivers.
• Emotional Support : Provide compassionate care and emotional support to patients and their families during difficult times.

Requirements

Certification in nursing or healthcare assistance

DOH license or eligibility letter (mandatory)

Experience in home care or elder care (preferred)

Strong interpersonal and patient care skills
